More and more school children are recommended to not carry a backpack and instead pulling them on trolleys in a hope to avoid long term spinal problems.

Here’s a 2010 study in Germany which included thirty-four school children between 6 and 8 years of age comparing the children’s posture while carrying backpacks vs. while pulling them on trolleys.

The results?  While both groups had asymmetric postures when carrying a backpack or when pulling a trolley, the trolley group, however, had extra stress due to spinal rotation.

The results suggest that carrying a backpack may be better than pulling a trolley when the weight is within recommended limits.

Comparison of the Posture of School Children
Carrying Backpacks Versus Pulling Them on Trolleys

FROM:   Clinical Chiropractic 2010 (Dec);   13 (4):   253–260

Johanna Schmidt, Sharon Docherty

It’s well understood that heavy backpacks are taking a heavy toll (excuse the pun!) on adolescent spines. A recent standing magnetic resonance imaging study by the Department of Orthopaedic Surgery, University of California, at San Diego revealed that: “Increasing backpack loads significantly compressed lumbar disc heights measured in the midline sagittal plane” and that: “student subjects reported significant increases in back pain, associated with increasing backpack loads from 4, to 8, and finally to 12 kgs of carried weight”.

This most recent novel study (see below) was performed with German primary school children. [2] Researchers wanted to determine whether giving the children rolling luggage, to carry their school books, might reduce spinal stress. As it turns out, what might work well for Mom at the airport does not work so well for Junior, lugging his books across uneven sidewalks, all the way to school. The authors had to conclude that: “This suggests that school children should use backpacks rather than trolleys when the weight is within recommended limits”.

The Abstract:

Objective   To investigate whether there is a difference in the posture of school children walking with a backpack versus pulling a trolley.

Design   Comparative, controlled, pilot trial.

Setting   “Grundschule Fallersleben” – primary school in Germany.

Subjects   Thirty-four school children between 6 and 8 years of age.

Methods   Initially, neutral posture was measured in a standing position. All children were then asked to walk a predetermined route without intervention for approximately 7 min. This was followed by walking the same route with either a backpack (n = 19) or trolley (n = 15). Deviations from neutral of the thoracic and lumbar spine (flexion, extension, lateral flexion and rotation) from the final 30 s of the imaging sequences were taken and analysed.

Results   Compared to unburdened walking, walking with a backpack led to a statistically significant (p = 0.05) increase in thoracic extension (3.91°, 95% CI = 3.35–4.46) and right lumbar lateral flexion (2.29°, 95% CI = -3.41 to -1.18), and a statistically significant decrease of lumbar flexion (2.2°, 95% CI = 0.34–4.06). In contrast, walking with a trolley increased extension (1.4°, 95% CI = 0.72–2.08), right lateral flexion (1.24°, 95% CI = -1.91 to -0.57) and right rotation (3.09°, 95% CI = -3.85 to -2.32) of the thoracic spine, and led to a statistically significant increase in left rotation (3.57°, 95% CI = 2.58–4.55) of the lumbar spine. Comparing the backpack and trolley groups showed a statistically significant (p = 0.05) increase in thoracic extension and right lumbar lateral flexion in the backpack group. Posture during trolley pulling was characterized by a statistically significant (p = 0.05) increase in right thoracic and left lumbar rotation.

Conclusion   Participants adopted asymmetric postures during walking with a backpack and pulling a trolley. However, the trolley group was characterised by spinal rotation which possibly adds an extra source of stress. This suggests that school children should use backpacks rather than trolleys when the weight is within recommended limits.

One of the fastest growing segments of the population today is centenarians, or 100-year old people. In 1980, there were approximately 17,000 people over the age of 100. Today there are approximately 80,000 people who have reached this milestone. The US Census Bureau estimates that by the time the baby boom generation reaches this landmark age, there will be more than 4.2 million centenarians in the US.  This trend holds true for other industrialized nations including Australia, Canada, and those in Europe. Many of us have noticed this trend first hand by having to cope with the challenges of our aging parents.

Unfortunately, many elderly people are unprepared for their extended life.  When today’s centenarians were born in 1905, the life expectancy for a newborn was approximately 50 years. Yet many have outlived their life expectancy by half a century. If these people knew they were going to live this long, do you think many of them would have made better health choices along the way?

We are getting the advanced notice that our parents and grandparents never received. What will you do with this information? Hopefully you will choose to make your health a priority now instead of waiting until it deteriorates to choose a healthy lifestyle. The truth is that we cannot control how long we are going to live.  We can, however, control our health choices. As people age, quality of life choices take on a higher priority. Values turn more toward the physical and physiological standard of living rather than the material standard of living.

After all, what good is a nice car if your back hurts too much to sit in the seat and enjoy it? or if you have a pretty smile but you can’t enjoy your food because your teeth hurt?  Who cares what your carpet looks like if you don’t have the energy to entertain? Does it matter how much jewelry you have if you are too sick to go out on the town?

Many people spend the first 50 years of their lives ignoring their health while they accumulate wealth, only to spend the next 50 years losing their wealth to buy back their health. Knowing that your longevity potential is 100 years gives you an opportunity to place your health at the top of your value scale.  Every minute of every day we are faced with choices that contribute to our quality of life both now and in the future.

We must choose between eating junk food and healthy food, brushing and flossing your teeth or being lazy, being a couch potato and exercising, stressing out or looking at the bright side, getting regular dental checkups and chiropractic adjustments or living with the consequences of poor spinal and oral health.

Starting today, make health choices that ensure that each day will be your best, and encourage your family to do the same.

After all, 100 years of healthy living is achieved one day at a time!  We are excited to be a part of that team for you and your family.

Stand taller for posture month

May 1, 2011 — May is National Correct Posture Month, and it seems mom was even right about standing up straight. Improving your posture is not just about looking good. Studies show people with better posture really are healthier and actually live longer.

From kids with backpacks or cane-carrying seniors — studies show poor posture is a major cause of back and neck pain for all ages, and over time often contributes to digestive and cardio-pulmonary problems.

posture2

Here’s three things to help keep you standing tall:

1. Take an annual posture picture to set the benchmark for your body. Ask a friend to take your picture from the front, back and side. Print them out, one to a sheet, and look at your body symmetry.

Just comparing posture pictures from year to year can be eye-opening. You can assess your alignment by putting a dot between your feet on the front and back view, and at your ankle on the side view. Then, fold each paper in half neatly at the dot.

Front and back view: The two halves of your body should be the same. If your head and/or torso is off to one side, or your arms are hanging differently, your posture is not symmetrical.

Side view: The line from your ankle should pass thru your shoulder and ear. If your head is way forward of that line, you may have a posture distortion called forward head posture (FHP).

File your posture picture and compare it to a new one next year.

2. Do posture strengthening exercises to maximize the benefit of any workout, or as a beginning exercise to get in shape. Yoga, Tai-Chi, and Pilates are all great for building body awareness and control.

Best posture stork: Train yourself to stand tall while building good posture by balancing on one foot.  First, stand tall with your best posture and then focus on staying tall and straight as you lift your thigh so your knee is at hip height.  Keep standing tall for 30 seconds on each side, focusing on keeping your body well aligned.

Vertical wall stand: Back up to a wall so your heels, buttocks, shoulders, and head all lightly touch the wall while you keep everything level, relaxed and aligned — and take three slow breaths, feeling your body’s best posture. Note: If you feel any areas of stress, get your posture checked by a professional.

3. Choose smart posture habits, Get a good chair, and change it’s position during the day. If you’re going to play videogames, exercise more than your thumbs with games with one that makes you move such as the motion sensing videogame consoles.

Invest in quality shoes, and consider using a shoe insert or orthotic, especially if you’ve had foot problems.

If you have back pain, neck soreness, or other posture problem — or want to find out how strengthening your posture can improve sports performance (ie golf, tennis, baseball) and wellness — help is available. Consult a chiropractor or health care professional trained to analyze posture and teach individualized posture exercise routines for pain management and wellness.

Your posture is how the world sees you, and a bit of effort at strengthening your posture is an anti-aging habit to make you look and feel better, avoid injury and exercise effectively to stay active and age well.

Spinal manipulation may provide broader and more significant long-term benefit for chronic spinal pain patients than acupuncture and medication.
Muller R, Giles LGF. Long-Term Follow-Up of a Randomized Clinical Trial Assessing the Efficacy of Medication, Acupuncture, and Spinal Manipulation for Chronic Mechanical Spinal Pain Syndromes. J Manipulative Physiol Ther 2005;28(1):3-11.
BACKGROUND
Chronic mechanical back pain and neck pain, which are often caused by an injury or disease, present a diagnostic and treatment challenge and pose a significant financial strain on the health care system. Some evidence shows that a multidisciplinary team approach to treating spinal pain results in high patient satisfaction. The evidence about the superiority of a particular treatment for non-specific chronic spinal pain is inconclusive.
THE OBJECTIVE of this study was to assess the long-term effect of medication, needle acupuncture and spinal manipulation on treating patients with chronic (> 13 weeks) spinal pain.
THIS STUDY conducted a one-year follow-up of a randomized clinical trial investigating the effectiveness of medication (Celebrex, Vioxx and/or acetaminophen), acupuncture or high-velocity low-amplitude spinal manipulation on treating chronic spinal pain. Sixty-nine patients were randomized into three treatment groups, receiving one type of treatment for nine weeks. The one-year follow-up was conducted through Oswestry Back Pain Index, Neck Disability Index, Short-Form-36 and Visual Analog Scales. The study analyzed the results of treatment of 40 patients who had received only one randomly allocated type of treatment.
RESULTS
Comparison of the initial and long-term follow-up questionnaires produced the following results:
• Only the group receiving spinal manipulation showed long-term treatment benefit, with five of the original seven improvements remaining statistically significant after one year.
• Only one of seven improvements remained statistically significant in each of the acupuncture and the medication treatment groups at follow-up.
CONCLUSION
For treating chronic spinal pain, spinal manipulation, if not contraindicated, may provide broader and more significant long-term benefits than acupuncture or pain-relief medication.

In both the United States and Canada, back pain contributes to significant absenteeism and lost productivity in the workplace. In Alberta, Canada, alone, on-the-job back injuries account for more than 25 percent of all Workers’ Compensation Board lost-time claims.

A recent article in the Calgary Herald discussed how patients that receive chiropractic care return to work faster and at less cost than with other forms of treatment. In addition, for employers, increased use of chiropractic care can reduce premium costs for benefits and contribute to improved productivity.

Furthermore, cumulative performance statistics over the past decade from the Alberta Workers’ Compensation Board indicate that chiropractic care is the most consistent and cost-effective treatment per claim.

Does maintained Spinal manipulation therapy for chronic non-specific low back pain result in better long term outcome?

Senna, Mohammed K.; Machaly, Shereen A.

Spine: POST ACCEPTANCE, 17 January 2011

Abstract

ABSTRACT:: Study Design.

A prospective single blinded placebo controlled study was conducted.Objective. to assess the effectiveness of spinal manipulation therapy (SMT) for the management of chronic non-specific low back pain (LBP) and to determine the effectiveness of maintenance SMT in long-term reduction of pain and disability levels associated with chronic low-back conditions after an initial phase of treatments.

Summary of background. SMT is a common treatment option for low back pain. Numerous clinical trials have attempted to evaluate its effectiveness for different subgroups of acute and chronic LBP but the efficacy of maintenance SMT in chronic non-specific LBP has not been studied.

Subjects and Methods. 60 patients with chronic, nonspecific LBP lasting at least 6 months were randomized to receive either

(1) 12 treatments of sham SMT over a one-month period,

(2) 12 treatments, consisting of SMT over a one-month period, but no treatments for the subsequent nine months, or

(3) 12 treatments over a one-month period, along with “maintenance spinal manipulation” every two weeks for the following nine months.

To determine any difference among therapies, we measured pain and disability scores, generic health status, and back-specific patient satisfaction at baseline and at 1-month, 4-month, 7-month and 10-month intervals.

Results: Patients in second and third groups experienced significantly lower pain and disability scores than first group at the end of 1-month period (P = 0.0027 and 0.0029 respectively).

However, only the third group that was given spinal manipulations during the follow-up period showed more improvement in pain and disability scores at the 10-month evaluation.

In the no maintained SMT group, however, the mean pain and disability scores returned back near to their pretreatment level.

Conclusion. SMT is effective for the treatment of chronic non specific LBP. To obtain long-term benefit, this study suggests maintenance spinal manipulations after the initial intensive manipulative therapy.

Back pain often ranks as one of the top reasons for employee absenteeism and “presenteeism” (being at work but not really doing any work!) costing companies millions of dollars.  Back pain afflicts approximately eight of every ten Americans during their lifetime.

Another recent study published in the Journal of Occupational & Environmental Medicine – March 2011, showed Chiropractic to be more effective than traditional medical care as well as physical therapy for patients with low back pain.

Do you or do you know of someone that has Low Back Pain? Why continue to suffer?

Elbow Pain (Tennis Elbow, Golf Elbow) and Chiropractic

By
Mark Studin DC, FASBE (C), DAAPM, DAAMLP

Tennis elbow, golf elbow, shooter’s elbow “is a condition where the outer part of the elbow becomes sore and tender. It is commonly associated with playing tennis or other racquet sports, though the injury can happen to almost anybody. The condition is also known as lateral epicondylitis (“inflammation of the outside elbow bone”), a misnomer as histologic studies have shown no inflammatory process. Other descriptions for tennis elbow are lateral epicondylosis, lateral epicondylalgia, or simply lateral elbow pain…The peak incidence is between 30 and 60 years of age historically” (Wikipedia, 2010, http://en.wikipedia.org/wiki/Tennis_elbow).

Symptoms include pain on the outer part of the elbow (lateral epicondyle), point tenderness over the lateral epicondyle – a prominent part of the bone on the outside of the elbow, gripping and movements of the wrist hurt, especially wrist extension and lifting movements, activities that use the muscles that extend the wrist (e.g. pouring a pitcher or gallon of milk, lifting with the palm down) are characteristically painful, and morning stiffness.1

In December of 2008, a research article reported on the effects of chiropractic cervical manipulation (adjustment) on the pain pressure threshold, or the minimal amount of pressure to cause pain, along with grip strength, or the amount of strength in the hand. The results of the research concluded that the application of a cervical spine adjustment produced an immediate bilateral (both sides) increase in pain pressure threshold (decreased pain) and an immediate pain free grip on the affected side.2

This is one of the reasons that both professional golf and tennis organizations maintain chiropractors at high profile events as it is reported that the scores of the players have improved with chiropractic care. However, this also explains why chiropractic has been helping in work and at home in everyday life with a similar diagnosis.

This study along with many others concludes that a drug-free approach of chiropractic care is one of the best solutions to elbow pain. To find a qualified doctor of chiropractic near you go to the US Chiropractic Directory at www.uschirodirectory.com and search your state.
